Serious Case Review: the Murder of Gemma Hayter.

Warwickshire Safeguarding Adults Board (WSAB) commissioned a panel to undertake a Serious Case Review (SCR) following the murder of Gemma Hayter, a young woman with learning disabilities, on 9th August 2010.

A referral for a serious case review was made by Warwickshire County Council Adult Health and Community Services on 1st September 2010. It was considered by a multi-agency meeting chaired by the Chair of the Partnership and accepted on 28th September 2010. The grounds for doing so were based on the information available at the time:

  • A vulnerable adult had died and abuse or neglect is known or suspected to be a factor in the death
  • The case gives rise to concerns about the way in which local professionals and/or services work together to safeguard vulnerable adults.
