Jumping barriers, riding challenges…Getting the transition to adult social care right

Description: Dimensions is one of the UK’s largest not-for-profit support providers and in this webinar they introduce High Greave, a purpose-built community of five individual homes and a support environment specifically designed to build independence and unpick behaviours of distress amongst young adults with autism and/or learning disabilities. High Greave highlights the importance of collaboration between commissioners, social workers, the NHS, and support providers, with families playing a key role in the transition process. The success stories of individuals who have made High Greave their home, with some avoiding ATU admission thanks to this supportive environment are shared. This is how transitions should be and we invite you to reimagine the process of transitioning to adult services.
